Plan ID tool
Medicare Plan ID Decoder
The H-number on your card or letter is not random. It identifies the CMS contract, the plan benefit package, and the segment. Decode it here, then check the 2027 status in your county.
How to read the ID
| Part | Example | What it identifies |
|---|---|---|
| Contract | H1234 | The CMS contract. H contracts are Medicare Advantage; S contracts are stand-alone Part D. |
| PBP | 001 | The plan benefit package. A contract can offer several PBPs with different benefits. |
| Segment | 000 | A benefit segment within the PBP, often used for employer or regional variations. |
The CMS CPSC county file used by this site stores the contract and PBP and treats the segment as 000. The crosswalk status is attached to the contract and PBP, not to a person.
Examples
H1234-001-000— Medicare Advantage contract H1234, PBP 001, segment 000.H1234-001— same contract and PBP, segment not printed.S1234-001— stand-alone Part D contract; not a Medicare Advantage plan.E1234-001— employer or union direct contract; often outside the CPSC county file.

Plan ID questions
What does H1234-001-000 mean?
The first part (H1234) is the CMS contract number. The second part (001) is the plan benefit package, or PBP. The third part (000) is the segment. The same contract can offer several PBPs and segments with different benefits.
Why does my letter show only H1234-001?
Some plans print the contract and PBP without the segment. In the CMS CPSC county file used by this site, the segment is treated as 000. Search for H1234-001 in the lookup tool; the county plan list will show the matching rows.
Is every plan ID in this site?
No. The CPSC files exclude most employer group plans, PACE plans, cost plans, and stand-alone Part D plans. If your ID is an S contract, it is a Part D plan, not a Medicare Advantage plan. Use Medicare Plan Finder for those.
Does a plan ID tell me whether the plan is leaving?
No. The plan ID identifies the contract, PBP, and segment. The 2027 status comes from the CMS crosswalk. Decode the ID here, then open the lookup tool with your ZIP code to see the status for your county.
